How to Register

  1. Complete the form below.

  2. Go to the Policy Handbook & Forms tab, complete the waiver form, read the handbook, and sign the acknowledgment form.

  3. Once we receive your registration, we will confirm with you and contact your caseworker to set the authorizations in place.

  4. After we receive the authorizations, we will contact you again to arrange activities and schedules.
